The services Well Health provides to its 13000 enrolled patients include:

The population that we serve is diverse and has a range of high and complex health and social needs. A community development approach is taken that sees community and engagement as cruical to the achievement of positive health outcomes and improved quality of life for our communities. This enables equitable access to primary health care services to ensure that, together; we improve and maintain our communities’ health and wellbeing status resulting in increased quality of life.
